NHS is currently seeking an Intensive Case Manager to join Carbondale, PA Team.

  • This position provides Intensive Case Management services to seriously and persistently mentally ill consumers with goals of reducing days of hospitalization, increasing independence, strengthening social supports, and emphasizing educational and vocational efforts. Such services include;assessment, service planning, linking, monitoring psychiatric status, support network building, problem resolution, obtaining resources, education, skill building and advocacy.

  • Bachelor Degree in psychology, social work, nursing, rehabilitation, counseling, or other human services field and 3 years mental health direct care experience

  • Or an RN license with 3 years direct care experience or 2 years supervisory experience.

  • Must have a valid driver's license and reliable transportation
